How to Get a Doctor's Note for Stress Leave in Ontario (and What to Do in the First 2 Weeks)
Buena Vita Therapy team
Registered Psychotherapists (RP), CRPO, Ontario

Book an appointment with your family doctor or a nurse practitioner, describe how stress or burnout is affecting your sleep, mood and ability to work, and ask for a medical certificate stating you cannot work and for roughly how long. Registered Psychotherapists cannot sign it. Ontario's 3 job-protected sick days need no note.
You finally did it. You stopped.
Maybe your doctor said the words out loud. Maybe you cried in the car before a Monday meeting and knew you could not walk in. Maybe your body made the decision for you.
Now you are home on burnout leave, and instead of relief, you feel a strange mix of guilt, fear and exhaustion. You check your work email "just once." You lie awake wondering what your team thinks. You tell yourself you should be resting, then feel guilty that rest does not feel restful.
If that is where you are right now, take a breath. Nothing is wrong with you. This is what the first days of a burnout leave often feel like.
This guide walks you through the first two weeks: what to rest from, what paperwork to handle, and how to start recovering so you do not go back to the same place. It is general information, not legal, financial or medical advice.
Why the First Two Weeks Matter So Much
The start of a leave sets the tone for everything that follows.
Spend it answering "quick questions" from work, and your nervous system never gets the message that it is safe to stand down. Spend it in panic about money and forms, and the stress simply changes address.
But use these two weeks well, and something shifts. Sleep slowly improves. The fog starts to lift. You begin to see what actually pushed you to this point, and what needs to change so it does not happen again.
You do not need to fix your whole life in fourteen days. You just need a few solid steps.
Week 1: Stop, Protect Your Rest, Handle the Essentials
Let yourself actually stop
For the first few days, your only job is to lower the pressure. Sleep when you can. Eat simple food. Go outside, even for ten minutes. Say no to plans that drain you.
If you feel restless or guilty, that is common. Burnout often comes from years of treating rest like something you have to earn. Rest is not a reward here. It is the treatment.
Put a boundary around work
Turn off work notifications on your phone. Set an out-of-office message. If you need to stay in touch with your employer, agree on one contact person and one way to reach you.
If saying this feels impossible, our workplace boundaries scripts give you words you can borrow.
Tell your employer, in writing if you can
You do not need to share your diagnosis or every detail. A short, factual note that you are on medical leave, and when you expect to update them, is usually enough. Keep a copy of everything you send and receive.
Your Leave Options in Ontario, in Plain Language
Every workplace is different, so start by asking HR (or checking your benefits booklet) what your employer offers. Here are the main options many people in Ontario look at. Rules can change, so always confirm the details on the official pages linked below.
Employer sick days or short-term disability
Many employers offer paid sick days or a short-term disability (STD) plan through group benefits. If yours does, it is usually the first place to start, and the insurer will often ask your doctor to complete forms.
Ontario's job-protected sick leave
Under the Employment Standards Act, most employees can take up to 3 days of unpaid, job-protected sick leave each calendar year for personal illness, injury or medical emergency. Since October 28, 2024, employers cannot require a medical note for these days.
Ontario's long-term illness leave
Since June 19, 2025, eligible employees who have worked for their employer for at least 13 consecutive weeks can take up to 27 weeks of unpaid, job-protected long-term illness leave in a 52-week period for a serious medical condition. It needs a certificate from a qualified health practitioner (a physician, registered nurse, including a nurse practitioner, or psychologist) and written notice to your employer.
EI sickness benefits
If you cannot work for medical reasons, EI sickness benefits can provide up to 26 weeks of financial support, at 55% of your earnings up to a weekly maximum. You need a medical certificate, and the Government of Canada advises applying as soon as possible after you stop working, because applying more than 4 weeks after your last day may mean losing benefits. That deadline is why this belongs in your first two weeks.
If you are unsure what applies to you, an HR representative, your union, or a free legal clinic in Ontario can help.
How to Get a Doctor's Note for Stress Leave in Ontario
A doctor's note (often called a medical certificate) is what most longer leaves, EI sickness benefits and short-term disability claims are built on. Here is how to approach it, step by step.
1. Book with your family doctor or a nurse practitioner
Your own family doctor or nurse practitioner is usually the best person to see, because they know your history. If you do not have one, a walk-in clinic or virtual clinic may be able to help. Ask for a longer appointment if you can, so you do not feel rushed.
2. Write down what is happening before you go
When you are exhausted, it is easy to say "I'm fine" the moment you sit down. Jot down a few notes: how long you have felt this way, how you are sleeping and eating, panic or crying, trouble concentrating, and specific ways work has become hard to manage. Be honest. You are not exaggerating by describing what is real.
3. Ask clearly for what you need
It is okay to say, "I don't think I can safely keep working right now. Can we talk about a medical leave, and can you complete a note or my insurer's forms?" Bring any forms from HR, your short-term disability insurer or Service Canada.
4. Know what the note usually says
You do not have to share your diagnosis with your employer. A note for your employer usually confirms that you cannot work for medical reasons and for approximately how long. Insurance and EI forms ask for more detail, and those go to the insurer or Service Canada, not your manager. Your practitioner may charge a fee for forms.
5. Where therapy fits
Registered Psychotherapists are not authorized to communicate a diagnosis in Ontario, so we do not sign medical certificates or disability forms, as the College of Registered Psychotherapists of Ontario explains. What we can do is support you through the leave, help you understand what led here, and, with your consent, share factual information about your attendance in therapy with your doctor.
Week 2: Start Recovering, Not Just Resting
By the second week, many people notice a small lift. Others feel worse for a few days, as the adrenaline wears off and the tiredness finally catches up. Both are normal.
Add gentle structure back in
Wake up around the same time. Move your body in a way that feels kind. Plan one small, nourishing thing each day: a walk, a call with a friend, a meal you actually enjoy.
Notice what drained you
Without judging yourself, start a simple list. Which tasks, people, expectations or hours took the most out of you? This list will matter a lot when it is time to plan your return.
Book support before you "feel ready"
Many people wait to start therapy until they have more energy. But a leave is one of the best times to begin. You finally have the space to look at the patterns that led here: the over-functioning, the trouble saying no, the self-worth tied to productivity.

Sometimes a leave reveals that the exhaustion runs deeper than work. If, after a couple of weeks of real rest, you still feel empty, hopeless, or uninterested in almost everything, it may be worth exploring whether depression is part of the picture.
Our guide on depression vs burnout explains the key differences, and our page on online depression therapy in Ontario explains how we can help.
Imagine Going Back Differently
Picture yourself a few months from now.
You are back at work, but it is not the same version of work. You log off at a reasonable hour and you do not spend the evening replaying the day. You say "I can take that on next week" without your heart racing.
You sleep. You have energy left for your partner, your kids, your friends. Weekends feel like weekends again.
And when the old pressure creeps back, you notice it early, and you know what to do.
We cannot promise exactly how recovery will unfold for you. Everyone's path is different. But a leave does not have to be a pause before the same burnout. With the right support, it can be the turning point.
Your leave can be the start of something better. The first step is a short, easy conversation.

Frequently Asked Questions
How do I get a doctor's note for stress leave in Ontario?
Book an appointment with your family doctor or a nurse practitioner, explain how stress is affecting your health and ability to work, and ask for a medical certificate stating you cannot work and for roughly how long. Bring any HR, insurer or EI forms with you.
How long can stress leave last in Ontario?
It depends on which leave applies. Ontario's sick leave is up to 3 unpaid, job-protected days a year. Long-term illness leave can last up to 27 weeks in a 52-week period for a serious medical condition. EI sickness benefits can pay up to 26 weeks. Short-term disability length depends on your plan.
Does burnout count for sick leave in Ontario?
Ontario's 3 sick days can be taken for personal illness, injury or medical emergency, and employers cannot require a medical note for them. For longer leaves, EI sickness benefits or disability claims, what matters is a medical certificate showing you cannot work for medical reasons.
Can I take a leave from work for burnout in Ontario?
Many people do. Options can include employer sick days or short-term disability, Ontario's job-protected sick leave and long-term illness leave, and EI sickness benefits. Eligibility depends on your situation, so check with HR and the official government pages.
Can a therapist give me a doctor's note for stress leave?
Registered Psychotherapists in Ontario cannot communicate a diagnosis or sign medical certificates. Your family doctor or a nurse practitioner usually completes these. Therapy can still be an important part of your recovery during the leave.
When should I apply for EI sickness benefits?
As soon as possible after you stop working. The Government of Canada notes that applying more than 4 weeks after your last day of work may mean losing benefits.
Should I start therapy while I am on leave?
For many people, yes. A leave gives you time and space to understand what led to burnout and to plan a healthier return, rather than going back to the same patterns.
You Are Allowed to Recover
You held it together for a long time. Longer than you should have had to.
Now you get to put some of that strength toward yourself.
You do not have to have it all figured out. You do not have to feel ready. You just have to take one small step.
Book a free 15-minute video consult. Ask your questions. See how it feels to talk to someone who understands burnout from the inside out. If it is not the right fit, that is completely fine.
